Alferso share external SSO - How to implement logout button?

My external SSO is completed. I'm able to login to Share with openAM user. However, I don't see the logout button.

I have 2 questions:

1. How to enable logout?

2. How to setup custom URL for logout?

I'm trying to resolve this for the past 4 days, but havent found a solution. Any help will be greatly appreciated.

This is reasonably complicated and depends on which Alfresco version you are using.
