checkboxes & radio buttons as input

I modified the userprofile form to capture some values but via radio buttons and checkboxes.
However the inputs aren't captured.
below is a snippet of what I tried:

However select boxes to work with this code:

Can someone please enlighten me as to what I'm doing wrong here.
